Q: When should someone consider home health care?
  • Before scheduled surgery or during hospitalization: We provide nursing support and therapy after you leave the hospital so you can regain your health or improve mobility.
  • If you have been newly diagnosed with a serious illness: We offer in-home monitoring, therapy and one-on-one instruction to help you manage your health successfully.
  • If an existing chronic condition causes a decline in health: We will help you control new or worsening symptoms with in-home care or instruction on how to manage your condition.
  • When you have a high risk of falling and injuring yourself: We can help you avoid falls by addressing your risks with personalized nursing or therapy.
Q: Who pays for home health care?

Medicare A and B cover 100% of qualified home health services, with no out-of-pocket co-pays and/or deductibles. Medicare HMOs, Medicaid and private insurance cover many home health services. However, coverage varies and may require patient co-pays and/or deductibles. For information about insurance coverage, call 1-866-846-8081.

Q: Who qualifies for home health care?

Medicare and Medicaid will cover home health care for individuals who are homebound due to health or mobility limitations and need recurring skilled nursing care and/or therapy, which may include physical, occupational and speech therapy. Other health insurance plans may have different requirements. Generally, home health services must be ordered by a physician or an advanced practice provider, such as a nurse practitioner or physician assistant.

Q: Where can someone receive care?

Individuals can receive home healthcare in the comfort of their own residence or the home of a friend or relative. We also provide services in assisted-living facilities and other types of senior housing.
